THC, the main psychoactive compound in cannabis, has just gained an important new role in mental health treatment. A study published on August 5, 2026, in the scientific journal Nature Medicine revealed that a THC-based medication demonstrated significant efficacy in reducing and even eliminating recurrent nightmares in patients with Post-Traumatic Stress Disorder (PTSD).

 

What PTSD is and why nightmares are a problem
 

Post-Traumatic Stress Disorder (PTSD) is a psychiatric condition that can develop after exposure to traumatic events such as violence, severe accidents, natural disasters, or war situations. Among the most debilitating symptoms of PTSD are recurrent trauma-related nightmares, which impair sleep quality and worsen psychological distress for patients.


Until recently, no medication was specifically approved to treat trauma-related nightmares in Germany, leaving many patients without effective therapeutic options. Antidepressants and blood pressure medications are commonly used, but they offer only partial relief and generally do not specifically target nightmares.

The study from Charité – Universitätsmedizin Berlin
 

Researchers from the German hospital Charité – Universitätsmedizin Berlin conducted a multicenter, randomized, double-blind, placebo-controlled clinical trial, considered the gold standard of clinical research.

Study Methodology

 

  • Participants: 171 adults diagnosed with PTSD and recurrent nightmares (mean age 37.9 ± 12.8 years; 79.3% women)


     

  • Treatment: Dronabinol (BX-1; $\Delta^9$-tetrahydrocannabinol/THC) or placebo administered once daily before bedtime


     

  • Duration: 10 weeks of continuous treatment


     

  • Dosage: Ranging between 2.5 mg and 15 mg per night


     

  • Design: 87 patients were randomized to dronabinol and 84 to placebo


     

  • Primary outcome: Change in item B2 score of the CAPS-IV (Clinician-Administered PTSD Scale for DSM-IV), which assesses nightmare frequency and intensity

Results: Significant reduction in nightmare frequency and intensity

The results published in Nature Medicine revealed statistically significant data regarding the efficacy of dronabinol in treating trauma-related nightmares.

  • Group difference: At week 10, the reduction in CAPS-IV B2 scores was significantly greater with dronabinol than with placebo (between-group difference of -1.50; 95% confidence interval -2.28 to -0.71; Cohen's d = 0.65; $P < 0.001$)


     

  • Absolute reduction: In the dronabinol-treated group, the mean nightmare burden decreased by 3.7 points on a scale of 0 to 8, whereas in the placebo group the reduction was 2.2 points


     

  • Complete elimination of nightmares: More than one-third (approximately 35–37%) of treated patients reported having no nightmares after the 10 weeks of treatment, compared to 15% in the placebo group


     

  • At least 50% reduction: An additional 21% of patients showed a reduction of at least half in nightmare frequency or intensity


     

  • Subjective improvement: About five in six patients (approximately 83%) in the treated group reported significant improvement in their health status


     

  • Response rate: More than half of the participants responded to treatment


     

Professor Stefan Röpke, lead researcher for the project at Charité's Department of Psychiatry and Neurosciences, emphasized that the results represent an important breakthrough for patients suffering from this particularly distressing symptom of PTSD.


 

How THC Acts on the Brain

Dronabinol acts on the endocannabinoid system, which is responsible for regulating functions such as sleep, stress, and emotional memory processing. The researchers hypothesize that the medication helps the brain reduce dream activity during REM sleep—the intense dreaming phases in which the brain processes experiences and regulates emotions—thereby alleviating nighttime stress responses.

 
It is important to highlight that the treatment effect was specifically targeted at nightmares and sleep quality, without conclusively altering the overall severity of PTSD or comorbid depression. This means dronabinol does not replace psychotherapy or other treatments for the disorder as a whole, but it offers relief for a particularly debilitating symptom.


 

Safety and Tolerability of the Treatment

One of the most relevant aspects of the study was dronabinol's safety profile.


 

Observed Adverse Events

  • Overall adverse events: Occurred in 78 patients (89.7%) receiving dronabinol and 64 (77.1%) receiving placebo


     

  • Discontinuation due to adverse events: Five patients (5.7%) in the dronabinol group and six (7.2%) in the placebo group discontinued treatment due to adverse events


     

  • Serious adverse events: Occurred in seven patients (8.0%) receiving dronabinol and in none receiving placebo


     

  • Most common adverse effects: Dizziness, headaches, and increased appetite, all of mild to moderate intensity, occurred more frequently in the dronabinol group


     

  • No withdrawal symptoms: Following treatment discontinuation, researchers observed no signs of dependence or withdrawal symptoms


     

The authors emphasize that the treatment does not correspond to smoked cannabis or unregulated products, reinforcing the importance of using pharmaceutical-grade medications with controlled dosages and medical supervision.
 
 
With proven action on the endocannabinoid system, THC solidifies its position in medical cannabis due to its powerful effect in relieving chronic pain, sleep disorders, and severe symptoms of PTSD.

 

Limitations and Next Steps in Research

Despite the promising results, the researchers highlight several limitations and questions that still need to be investigated:


 

  • Duration of effect: It remains an open question whether the benefit persists beyond the 10 weeks of treatment or whether tolerance develops with prolonged use


     

  • Long-term studies: The authors advocate for further research to evaluate the effects of treatment over longer periods


     

  • Regulatory approval: There is no specific approval for this indication in Germany, although prescription is possible in individual cases at physician discretion since the 2017 "Cannabis as Medicine" Act

Medical Cannabis in the Context of PTSD

This study adds to growing evidence regarding the therapeutic potential of cannabinoids in managing PTSD. Previous research had already suggested that cannabinoids may help mitigate symptoms such as anxiety, sleep disturbances, and recurrent nightmare episodes.


 

Cannabis-based medications are already used in pain management, particularly when standard therapies are insufficiently effective or cause intolerable side effects. In Germany, access became easier after the "Cannabis as Medicine" Act came into effect in 2017, expanding the medical and scientific use of plant-derived or synthetically produced cannabinoids.

The results of this study offer hope to millions of people worldwide suffering from trauma-related nightmares. For patients who do not respond to conventional treatments or who seek therapeutic alternatives, dronabinol may represent a valid option when prescribed and monitored by qualified healthcare professionals.


 

However, it is crucial for patients to understand that:


 

  • The medication specifically treats nightmares, not PTSD as a whole


     

  • Psychotherapy remains essential for managing the disorder


     

  • Use must always be supervised by medical professionals, with controlled dosages


     

  • Unregulated cannabis products do not offer the same guarantees of safety and efficacy


     

Future Perspectives

The new data strengthen the rationale for using cannabinoids to manage specific PTSD symptoms and could influence future regulatory decisions regarding the approval of THC-based medications for this indication. Meanwhile, research continues, with ongoing studies evaluating long-term effects and potential applications in other patient populations.


 

The researchers' next steps include determining whether cannabinoid use remains effective and safe over the long term or whether tolerance develops over time.
